Nasus is a near unstoppable force, but if for instance, Vayne managed to get a bolt, through his heart and out again. We know him to be mortal, we only don't know where the hell his heart is And that's another thing. Non-humans. Do yordles have the heart on similar places? Does Cho'gath even have a heart? Even some humans are born "mirrored" So let's consider that. Each non-human has different fatal zones, and while champions MAY know them, and summoners SHOULD, any non-league people won't.
This is also another very good point. What if Cho'gath doesn't have a heart? What if he has four redundant ones? Evolution says that's fucking retarded? Well that's our Earth-style evolution speaking. Evolution in the Void might be entirely different in that additional hearts helped them brave the terrors of the Void by being able to send blood through their bodies faster? Cephalopods have multiple hearts already. Why can't Cho have seven? Maybe they pulled Blade Runner and have a skeletal system encasing their heart.
What if Skarner doesn't have a heart but a Crystalline substitute? Swords aren't going to touch that. What about Malphite? He's just a giant sack of rocks. I highly doubt he has a heart either.
There are definitely weakpoints one may strike for massive damage, but you're going to have to wait for them to light up in vibrant colours so that you'll know to flip them over and such.
Oh, and the luck thing too. What if Nasus trips on his brother's dead body and impales himself on his deceased sibling's weapon as Nasus was just about to slap the shit out of some random grunt? That grunt's going to get like, 1,000,000,000 experience points and enough loot to last him the rest of his life.
But that counts as the soldier having everything going for him and Nasus just being shit out of luck. The planets literally aligned just so the soldier would win. I would put that under considerable effort / not falling under "all else equal."

I re-iterate that the best way to resolve IC conflict is with OOC discussion about the outcome of the encounter. If Nasus' player and Renekton's player mutually decide that, for the sake of the plot, Renekton should die (not that they should!), then Nasus should best Renekton in combat. And, out of respect for Renekton's abilities, the gator should give him a run for his money.
If one does not feel comfortable or confident enough to discuss these things with other participants prior to the event, one should not participate in IC combat.

It's one thing to discuss results and another entirely to know relative power levels, imo.
Hypothetically speaking, I'm fighting someone with a gun (The pole weapon, not a rifle). She lunges towards me, jumping into the air with the weapon above her head and ready to slam it into me.
Situation A (I'm a rank higher)
Katarina runs her tongue along her teeth; lips curling into a smile. The assassin steels her ground, entering a combative stance as her hands grasp the hilts of her two swords. The Noxian assassin draws first with her dominant hand - then with her left. Sparks fly as the steel of her bladed weapon makes contact with the polished staff- redirecting the polearm's projected path when her other makes contact with the surface as well. A sickening screech as Katarina jumps upwards, the second blade grinding along the length of her opponent's weapon as the assassin cuts it upwards towards her mark's hands.
Situation B (We're equals)
The assassin furrows her brows, immediately drawing her blade with the longest reach in her right as her left draws a dagger. Katarina takes a step forward, bringing her blade up to meet the staff. The Noxian grunts, air being forced through gritted teeth. A brilliant shimmer as her sword embeds itself into the ground a few feet away. She lifts her right foot and eases herself back ever so slightly, dodging the staff as her marginally successful parry followed through; the force of the blow having knocked the blade out of her hands. Utilizing the momentum, Katarina spins, slamming her right boot down into the soil and anchoring herself as her left foot kicks out towards her mark.
Situation C (She's stronger)
Katarina breathed in, green orbs tracking the mark as it jumped into the air. She was fast. The assassin exhaled, regaining her sense of calm as her mind churned. Counter. Parry. The assassin had drawn her knife almost on instinct as she crouched. There wouldn't be enough time. Dodge. The red haired Noxian kicked off against the ground, jumping forwards in a tumble even as she heard the sickening crunch of the ground splitting where she had stood moments before. Her free hand slammed into the ground, pushing off as she somersaulted through the air; throwing the dagger she had drawn moments before- the hand she had previously used to push herself off gravitated towards and drew her longer blade as her leather boots once again made contact with the soil. Katarina immediately spun, her free hand grasping another dagger as she dug her katana into the ground to quell her momentum. Counter. She threw the other dagger as she sprinted forward, unearthing the glimmering sword as she dashed towards her mark.
Apologies for shoddy writing. It's like 6 AM here >A<;;
But yeah, I personally would react slightly differently to each despite it being same opponent depending on their power. If they're weaker I can be a lot more aggressive, relying on my superior stats to force them on their heels. If they're the same then I will respond defensively or offensively, trying to edge the line between the two based on my partner's actions. If they're better, then I'll have to fight extremely cautiously, and so on. ._.~
